Parkland RCA Overview

Cause Analysis is:
•   Parkland Methodology for reviewing cases which may/may not result in sentinel/serious safety event.

•  Systematic process built to identify and prioritize system vulnerabilities so they can be eliminated or mitigated and remediated to achieve the ultimate objective of improving patient safety.

•  Risk-based approach focused on harm potential and the probability of patient impact. 

Goals:
•  Improve safety of patient care across the continuum of care

•  Focus on systems and not professional practice

•  Prevent recurrence of similar events
