You should discuss an event or near miss with your attending or supervisor as soon as possible whenever:
• A patient’s care, safety, or well-being might be affected – for example, in the case of a serious error, complication, or unexpected outcome.
• You are unsure of the next steps or require guidance on how to manage the situation.
• You believe the event has broader systemic implications that may need leadership involvement.
While your attending or direct supervisor is your first point of contact, it’s generally advisable to discuss the event with your Program Director if:
• You need help understanding residency or fellowship implications of the event—particularly if it may affect training requirements, evaluation, or progression.
• You require additional support or resources, whether that’s counseling, remediation, or help navigating complex hospital processes like disclosure or patient safety reviews.
