FailSafe serves as your guide to understand what an Event Report or Root Cause Analysis is, and how you can safely report an event at Parkland. Several types of patient safety events could be reported through this application, such as, medication errors, falls, needle sticks, near misses (close calls), etc. Its purpose is to help increase awareness and engage everyone in effectively reporting patient safety events, and through critical event analysis enhancing our ability to deliver consistently safe, high-quality patient care.
Coming Summer of 2025
While this web app provides event reporting information for Parkland residents, we are excited to announce that a full native mobile app is currently in development for multiple locations including Clements, Parkland, Scottish Rite, VA, and Childrens. It will be available for testing in July 2025. This upcoming FailSafe app is designed to improve patient safety by educating and promoting the reporting process. The app will serve as a mobile-based quick reference tool providing easy access to Quality Improvement Methodologies utilizing an intuitive interface and additional features that support thorough and efficient reporting of patient safety events. Watch for news of the upcoming full FailSafe app from the Office of Quality Safety & Outcomes Education.
